U.S. Chemical Safety Board safety video discussing how improperly installed or modified pressure vessels have led to a number of serious chemical accidents.
In this video, the CSB Chairman, John Bresland, delivers a very direct warning about the dangers posed by inadequate pressure vessel safeguards. Although the risks associated with pressure vessels should never be underestimated, he pays special attention to the dangers surrounding the installation, welding, and modification of pressure vessels. Additionally, he emphasizes the importance of effectice pressure relief systems and provides several actual incidents as examples from which the industry can learn.
About the Chemical Safety Board
The CSB is an independent federal agency charged with investigating serious chemical accidents. CSB investigations examine all aspects of chemical accidents, including physical causes such as equipment failure as well as inadequacies in regulations, industry standards, and safety management systems. The Board does not issue citations or fines but makes safety recommendations to companies, industry organizations, labor groups, and regulatory agencies such as OSHA and EPA.
