US Chemical Safety Board, November 10, 2015

The U.S. Chemical Safety Board (CSB) will convene a public meeting on November 23, 2015, starting at 1 p.m. EST in Washington, DC, at the CSB’s office located at 1750 Pennsylvania Avenue, NW, Suite 910. The meeting is free and open to the public.

The Board will discuss investigations and operational activities through the end of the calendar year and will also provide an update on CSB audits conducted by the EPA Inspector General. The agenda will also include a discussion of future public meetings to explore worker fatigue and process safety management issues, as well as a staff presentation and Board vote on a recommendation to the BP Global Executive Board of Directors to implement an incident reporting program. The recommendation was issued as part of the investigation report of the BP America Refinery explosion in Texas City, Texas, in March 2005.

An opportunity for public comment will be provided. The time provided for public statements will depend upon the number of people who wish to speak. Speakers should assume that their presentations will be limited to three minutes or less, but commenters may submit written statements for the record.

A conference call line will be provided for those who cannot attend in person.  Please use the following dial-in number to join the conference: 1-888-862-6557, confirmation number 41154160.

The CSB is an independent federal agency charged with investigating accidents and hazards that result, or may result, in the catastrophic release of extremely hazardous substances. The agency’s Board Members are appointed by the President and confirmed by the Senate. CSB investigations look into all aspects of chemical accidents and hazards, including physical causes such as equipment failure as well as inadequacies in regulations, industry standards, and safety management systems.
